A case of primary cystoad enocarcinoma of the liver


Abstract
Hepatoma or liver cell carcinoma is common disease particularly among the Asians but the primary cystoadenocarcinoma arising from the liver is an extremely rare condition and only a few cases were reported so far in the literature.
A 56-year-old female patient was admitted with chief complaints of slowly growing mass in the upper abdomen and discomfort in the area. The patient was also found to be diabetic. Liver scanning showed a large cold area completely replacing the left lobe of the liver. Liver function test was normal. Fasting blood sugar was 1.81 mg/100ml. Laparotomy under general anesthesia revealed a large cystic mass measuring 16 by 13 by 10 cm arising from left lobe of the liver. All other intraabdominal organs were negative and the remainder of the liver was obviously normal: There was no evidence of lymphnode involvement of the tumor.
Left hepatic lobectomy together with the tumor was done. The resected mass weighed 1, 150 gm. on cutting the mass, clear amber colored fluid flow out. The inner surface of the cyst was smooth except an area about 8cm in diameter where papillary hard growth was found. Biopsy from this area showed moderately differentiated papillary adenocarcinoma. The patient had smooth postoperative recovery and was discharged two weeks later. She is well without evidence of recurrence one and half years after the surgery. The diabetes also disappeared and urine is pers-istently negative for sugar.
